TCPA Consent & Lead Form Disclosure

Econo-Wise Insurance Agency Inc. d/b/a Oklahoma Contractor Insurance

This document establishes the TCPA consent language, lead form disclosure requirements, and opt-out procedures for the Oklahoma Contractor Insurance website (oklahomacontractorinsurance.com), operated by Econo-Wise Insurance Agency Inc. (NPN: 7861499).

Part A: Required Consent Language for All Lead Forms

The following consent language appears clearly and conspicuously on or immediately adjacent to every quote request, contact, or lead capture form on the website, positioned immediately above the submit button:

By clicking "Get My Free Quote" (or "Submit"), I consent to be contacted by Econo-Wise Insurance Agency Inc. and its licensed agents and insurance carrier partners regarding commercial insurance products, using automatic telephone dialing systems, pre-recorded or artificial voice messages, and/or text messages (SMS/MMS), at the phone number(s) I have provided, even if my number is on a state or federal Do-Not-Call registry. Standard message and data rates may apply. I understand that consent is not a condition of purchasing any insurance product and that I may revoke consent at any time. I also consent to Econo-Wise's Privacy Policy and Terms of Service.

Part B: Display Requirements

To be enforceable, the TCPA consent language must be displayed in a manner that satisfies the following requirements:

Part C: Record Keeping Requirements

For each submitted lead, the following data must be captured and retained for a minimum of four (4) years:

  • Date and timestamp of form submission (UTC)
  • IP address of the submitting user
  • Exact version of consent language displayed at time of submission
  • URL of the page on which the form was submitted
  • All fields submitted (name, phone, email, etc.)

This data constitutes the evidentiary record of TCPA consent. It is stored in a tamper-evident log and is retrievable in the event of a regulatory inquiry or litigation.

Part D: Opt-Out and Revocation Procedures

Phone / SMS Opt-Out

Consumers may revoke TCPA consent for calls and texts at any time by:

  • Replying STOP, UNSUBSCRIBE, CANCEL, END, or QUIT to any text message
  • Calling (405) 682-6699 and requesting removal from the call list
  • Emailing [email protected] with the subject line "Opt Out"

Opt-out requests must be honored within 10 business days. Once a consumer opts out, their number is added to the agency's internal Do-Not-Call list and no further automated outreach may occur.

Email Opt-Out (CAN-SPAM)

Every marketing email must include:

  • A functioning one-click unsubscribe link
  • The physical mailing address: 2705 SW 29th Street, Oklahoma City, OK 73119
  • Clear identification of Econo-Wise Insurance Agency Inc. as the sender

Unsubscribe requests must be honored within 10 business days.

Part E: Carrier Partner Sharing Disclosure

When a consumer submits a quote request, their information may be shared with insurance carriers participating in our quote platform for the purpose of generating insurance quotes. Consumers should be aware that:

  • Carriers may contact them independently using the contact information provided
  • Each carrier's contact practices are governed by its own TCPA consent processes
  • Our consent language covers contact by Econo-Wise and its carrier partners

The following fields are captured in our consent log to satisfy evidentiary requirements:
